theme type binder print or type:	state,	award and applicant's name with
complete address. On combined title and index page repeat information on cover of binder. ADD name	of president or presidents (#1), number	of	members
and list of items by page number of contents of	entry. DESCRIPTION	of	project1
when and where activity started; scope and accomplishment; involvement of members, youth and community; dated publicity; before and after pictures; landscape plans (#1 and 3); letters of appreciation and/or any other evidence required. Please limit pages to 16, if possible. Both sides of paper may be used. All script should be typed - neat and concise. NO LOOSE ITEMS accepted. Include addressed mailing .label with correct amount of postage for return of book.
